Section Plc 1605.04 - Information Required for Registration Renewal Application

In addition to the information required by Plc 308.06, the applicant for renewal of a nurse agency registration shall provide the following information:

(a) The estimated number of licensed medical personnel the applicant expects to have available for assignments to clients, as a range and as an average.
