Section Plc 1409.03 - Conditions of License Suspension or Revocation(a) If a license is suspended, the licensee shall prepare and submit a plan of correction to address each violation that provided a basis for the disciplinary proceeding.(b) The executive director shall review the submitted plan of correction and: (1) Approve the plan, if it: a. Addresses each violation that provided a basis for the disciplinary proceeding; andb. Identifies one or more specific actions that the licensee shall take to correct each violation and specifies a deadline for taking each action that is reasonable given the nature of the action to be taken; or(2) Return the plan to the licensee for revision and resubmission if the plan does not meet the requirements for approval specified in (1), above.(c) A licensee whose license was suspended may request the license to be reactivated after the conditions specified in the suspension order have been met in accordance with Plc 1407.12(a).(d) If a license is revoked, any subsequent application for licensure shall include a plan of correction that addresses each violation that provided a basis for the disciplinary proceeding that resulted in the revocation.(e) An individual whose license has been revoked and who wishes to reapply for a license shall apply as for reinstatement of the license in accordance with Plc 1407.12(b).N.H.
