The board shall fully reinstate licenses lapsed for more than 70 days if:
(a) The licenses: (1) Have been lapsed for no more than 4 years; or(2) Have been lapsed for over 4 years but the reinstatement applicants have been active in the profession as defined in Phy 401.01 for the just preceding 4 years;(b) The reinstatement applicants have not violated RSA 328-A or RSA 328-F;(c) The reinstatement applicants have not demonstrated poor moral character as evidenced by: (1) The answers to the "yes-no" questions on the confidential and non-confidential parts of the reinstatement application form;(2) Any documents giving detailed reports of the relevant circumstances related to answers to such starred questions;(3) Any criminal offender record reports received by the board in compliance with Phy 403.08(e); and(4) Any verifications received by the board in compliance with Phy 403.08(f);(d) The reinstatement applicants have paid the reinstatement fee set forth in Ahp 301.02(a);(e) The reinstatement applicants have submitted:(1) A completed dated, signed and notarized reinstatement application form as further described in Phy 403.07; and(2) The supporting materials described in Phy 403.08; and(f) The reinstatement applicant has taken and passed the jurisprudence examination as described in Phy 303.07.N.H.
