Section Phy 402.07 - Required Documents Applicants for renewal of their licenses shall submit, or arrange for the board to receive, the following materials:
(a) A detailed report of the relevant circumstances if any of the answers to questions (e)(2) on part one of the application form is in the affirmative;(b) On a separate sheet, a detailed report of the relevant circumstances if any of the answers to questions (m) on part 2 of the application form is in the affirmative;(c) A completed work history form provided by the board; and(d) The passing scores on the most recently required jurisprudence exam as described in Phy 402.06.N.H. Admin. Code § Phy 402.07
