Section Pas 309.01 - Tramway Incident Report Form
(a) The "Tramway Incident Report" form shall include the following:
(1) Date of report;
(2) Incident specifics, including:
a. Name of Ski Area and Tramway registration number;
b. Type of lift on which incident occurred;
c. Date and time of incident;
d. Specific location of incident;
e. Name of ski area owner;
f. Mailing address; and
g. Telephone number;
(3) Carrier number;
(4) Type of passenger as follows:
a. Skier;
b. Snowboarder;
c. Foot passenger; or
d. Other;
(5) Each person injured or killed, including:
a. Name, address, and age; and
b. Detailed description of each person's injuries;
(6) Weather conditions, temperature, visibility, uphill snow conditions, and wind conditions;
(7) Name(s) and age(s) of attendants and lift operator in charge of tramway at the time of the incident;
(8) A brief description of the incident;
(9) Names and addresses of all witnesses;
(10) Person's name and address who performed the preoperational inspection of the tramway on the date of incident;
(11) If the injured person was treated at the scene of the incident, the name(s) of persons who administered treatment;
(12) Name of hospital or doctor's office where injured person was taken,if known;
(13) Detailed statement describing any lift system failure that contributed to the incident; and
(14) Signature of owner or operator.
