Section Pari 606.03 - Stable Name Application Form

On a form provided by the commission, an applicant for a stable name license shall supply the following:

(a) The year for which the applicant is applying for a license;

(b) The stable name;

(c) A list of the names and address of all individuals, corporations and partnerships using the stable name in (b) above;

(d) The type of license held by each individual, corporation and partnership listed pursuant to (c) above;

(e) Whether any of the individuals, corporations and partnerships listed pursuant to (c) above are presently under suspension, set down, ruled off or otherwise debarred from racing by any racing body or other recognized harness racing authority in the U.S. or elsewhere;

(f) If the answer to (e) above is yes, state all particulars of the ruling and the offense charged;

(g) Whether the stable name has been registered with any other racing association or commission, and if so, the name of the racing association or commission and the registration date;

(h) A list of all horses racing under the stable name;

(i) The applicant's signature, or, if the applicant is a corporation or other form of association, the signature of the applicant's duly authorized representative, on a statement preprinted on the form stating that:

(1) The applicant agrees to abide by the rules of the New Hampshire racing and charitable gamingcommission and the laws of the State of New Hampshire relative to greyhound racing, horse racing and pari-mutuel wagering;

(2) The information provided on the application form is true, accurate and complete; and

(3) The applicant acknowledges that, pursuant to RSA 641:3, making a false statement on this application form is punishable as a crime; and

(j) The date of the signature described in (i) above.
