Section Pari 605.34 - Waiver of Use of Application Form for Occupational Licensees

The commission shall grant an applicant a waiver from the use of the form described in Pari 605.05 provided that the applicant submits to the commission any occupational license application form produced by the ARCI, including:

(a) A written submission of any additional information required by the form specified in Pari 605.05;

(b) The applicable supplemental form specified Pari 605.07 through Pari 605.27; and

(c) The items required by Pari 605.03(a)(3) through (a) (4).
