Section Pari 605.22 - Supplemental Application Form for Horse Racing Chief of Security or Security PersonnelOn a form provided by the commission, an applicant for an occupational license as a horse racing chief of security or as security personnel shall, in addition to the application form specified in Pari 605.05, supply the following:
(a) The name of the applicant as it appears on the form specified in Pari 605.05;(b) The name, address and telephone number of the person, corporation or association licensed under RSA 284:15 recommending the applicant for the occupational license;(c) Whether or not the applicant possesses a valid New Hampshire security guard license and if so, the license number;(d) If the answer to (c) above in no, whether or not the applicant has applied for such a license;(e) Whether or not the applicant holds at least an associate's degree in a field related to criminal justice;(f) If the answer to (e) above is yes:(1) The name and address of the school from which the degree was obtained;(2) The type of degree held and the date awarded; and(3) The field of study in which the degree was issued;(g) Whether or not the applicant has at least 5 years of experience in law enforcement or security;(h) If the answer to (g) above is yes: (1) A description of the applicant's employment history, including the name of each employer, job title and dates of employment; and(2) A description of any additional experience in law enforcement or security, including the dates when that experience was obtained;(i) The applicant's signature on the statement set forth in Pari 605.28; and(j) The date of the signature described in (i) above.N.H.
