Section Nuh 401.04 - Reinstatement Application

The applicant for reinstatement shall fill out an application for reinstatement form supplied by the board and supply the following:

(a) Full name of the applicant;

(b) Current home address of the applicant;

(c) Current place of employment of the applicant;

(d) Current business address of the applicant;

(e) Business telephone number of the applicant;

(f) A list of states in which the applicant holds license(s), and verification that the licensee's license is in good standing provided by all states where the applicant is or was licensed;

(g) Whether the applicant has ever been disciplined in any state;

(h) The signature of the applicant;

(i) The date the applicant signed the application;

(j) A photograph of the applicant;

(k) Proof of 40 CEU clock hours earned in programs approved pursuant to Nuh 402.03; and

(l) Reason why applicant failed to renew his/her license.
