Section Mid 402.06 - Effect of Signature

The effect of the signature required by Mid 402.05(b) shall be that the applicant for renewal of certification:

(a) States that, to the best of his or her knowledge and belief, the information provided on the renewal application form and the documentation provided to support the renewal application are accurate, complete and unaltered; and

(b) Acknowledges that, pursuant to RSA 641:3, the intentional making of a false statement on the renewal application form is punishable as a misdemeanor.
