Section Lab 507.05 - Request for Pre Approval of Medical Hospital or Remedial Care(a) When pre approval of medical, hospital or remedial care in accordance with RSA 281-A:23 is required, the injured employee shall obtain a written statement from the medical provider containing the following: (1) The nature of the proposed care;(2) How the proposed care is required to address problems that are the result of the work-related injury;(3) The anticipated benefit of the care; and(4) An estimate of the cost for the care.(b) A copy of the statement from the medical provider shall be sent to both the insurance carrier and the department of labor. The carrier shall have 20 days to evaluate the request and decide if it will approve the requested care.(c) If the carrier denies the requested care or does not respond within the stated time frame, the injured worker shall contact the department, in writing, to request a hearing.N.H.
