Section Ins 4003.01 - Annual Registration Requirement(a) Each carrier and each third-party administrator shall submit a completed NHCHIS registration form, available at, to the department or its designee by March 15 of every calendar year. (b) Carriers and third-party administrators shall notify the department or its designee within 30 days of changes to any of the annual NHCHIS registration information.(c) Carriers and third-party administrators shall notify the department or its designee of any changes to the individual contact information submitted on the NHCHIS registration form as soon as possible, but no later than 30 days after a reassignment occurs.N.H.
