Section Ins 2702.08 - Availability of Plan Information(a) Criteria for medical necessity determinations. The criteria for medical necessity determinations made under group health insurance coverage issued to a large group employer with respect to mental health or substance use disorder benefits shall be made available by the health insurance issuer offering such coverage to any current or potential participant, beneficiary, or contracting provider upon request.(b) Reasons for any denial. The reason for any denial under group health insurance coverage or reimbursement or payment for services issued to a large group employer with respect to mental health or substance use disorder benefits in the case of any participant or beneficiary shall be made available by the health insurance issuer offering such coverage to the participant or beneficiary.N.H.
