Section Ins 2001.09 - Fees(a) The application fee that shall accompany the application for a medical utilization review license submitted pursuant to Ins 2001.04(b) shall be $500.00.(b) The annual renewal fee that shall accompany an application for the renewal of a medical utilization review license submitted pursuant to Ins 2001.05(a) shall be $100.(c) Fees payable pursuant to this section shall be payable to the State of New Hampshire.N.H.
