N.H. Admin. Code § Ins 1303.06

Section Ins 1303.06 - Course Completion Certificate

Each sponsoring organization shall provide to each student a course completion certificate for each course receiving continuing education credit containing the following:

(a) Sponsoring organization;

(b) Course number;

(c) Course title;

(d) Date of course;

(e) Total continuing education hours;

(f) Name of student; and

(g) Certification by sponsoring organization.
