Section He-W 577.04 - Covered Services

Laboratory services shall be covered when:

(a) Ordered and provided under the direction of a physician or other licensed practitioner within the scope of his or her practice; or

(b) Ordered by a physician but provided by a referral laboratory in accordance with 42 CFR 440.30.
