Section He-W 547.06 - Required Documentation(a) Providers shall document home visit referrals in the recipient's medical record.(b) Providers conducting home visits shall document the results of newborn health assessments, health promotion and anticipatory guidance, describing the assessment results and the provision of the services provided, and report them to the child's primary health care provider within 5 business days of the home visit.(c) Providers conducting home visits shall document the results of the maternal postpartum assessments, describing the assessment results, and report the results to the mother's primary health care provider or ob-gyn provider within 5 business days of the home visit.N.H.
