Cost effectiveness shall be determined by the department applying the methodology set forth in Chapter 3, Section 3910, Medicaid Payments for Recipients under Group Health Plans, of the federal Centers for Medicare and Medicaid Services (CMS), State Medicaid Manual, available as noted in Appendix A.
