Section He-P 4035.60 - Full Inspection Servicing for Teletherapy and Gamma Stereotactic Radiosurgery Units(a) A licensee shall have each teletherapy unit and gamma stereotactic radiosurgery unit fully inspected and serviced during each source replacement to assure proper functioning of the source exposure mechanism and other safety components. The interval between each full-inspection servicing shall not exceed 5 years for each teletherapy unit and shall not exceed 7 years for each gamma stereotactic radiosurgery unit.(b) This inspection and servicing shall only be performed by persons specifically licensed to do so by DHHS/RHS, an agreement state, or the Nuclear Regulatory Commission.(c) A licensee shall maintain a record of the full inspection servicing for teletherapy and gamma stereotactic radiosurgery units for the duration of use of the unit.(d) The record required in He-P 4035.60(c) shall contain:(1) The inspector's name;(2) The inspector's radioactive materials license number;(3) The date of inspection;(4) The manufacturer's name and model number and serial number for both the treatment unit and source;(5) A list of components inspected;(6) A list of components serviced and the type of service;(7) A list of components replaced; and(8) The signature of the inspector.N.H.
