Section He-P 4035.06 - Notifications

A licensee shall notify the DHHS/RHS in writing within 30 days when an authorized user, an authorized nuclear pharmacist, a radiation safety officer, an associate radiation safety officer, an authorized medical physicist, or ophthalmic physicist permanently discontinues performance of duties under the license, or has a name change, has appointed a temporary radiation safety officer as provided in He-P 4035.10(d), or has appointed an associate radiation safety officer as provided in He-P 4035.10(c).
