Section He-P 2104.06 - Change in Name of a Beverage or Bottled Water Facility(a) When a license holder intends to change the name of a bottled water or beverage facility, that license holder shall submit a written request to the department for a new license at least 30 days prior to the intended date of change in name.(b) The written request shall include: (1) The reason for requesting a new license;(2) The name of the bottled water or beverage facility as it appears on the existing license;(3) The name of the bottled water or beverage facility as the license holder requests it to appear on the new license; and(4) The date upon which the change in name is intended to occur.(c) Following receipt of the items required by (b) above, the commissioner shall issue a revised license reflecting the change in name. The establishment number and expiration date shall remain the same as it was on the immediately preceding license.N.H. Admin. Code § He-P 2104.06
