Section He-E 503.02 - Definitions
(a) "Alzheimer's disease and related disorders (ADRD)," means "ADRD" pursuant to RSA 161-F:66, I, namely "a condition which receives a differential diagnosis of Alzheimer's disease or a similar irreversible dementia."
(b) "Adult day program" means a facility licensed under RSA 151 and He-P 818 which provides the following services to eligible participants in accordance with a care plan:
(1) Administrative services;
(2) Personal care services;
(3) Health and safety; and
(4) Dietary.
(c) "Bureau" means New Hampshire's department of health and human services' bureau of elderly and adult services.
(d) "Care recipient" means any individual age 18 or older who is unable to attend to his daily needs due to ADRD, without the assistance or regular supervision of the primary caregiver.
(e) "Contract agency" means the agency contracted with the bureau to coordinate ADRD respite care services in accordance with He-E 503.
(f) "Nursing facility" means an institution or distinct part of an institution that is:
(1) Participating in the medicaid program;
(2) Meeting the requirements of Section 1919 of the Social Security Act 42 USC 1396r;
(3) Not primarily an institution for mental diseases or an institution for the mentally retarded or individuals with related conditions; and
(4) Providing one or more of the following:
a. Skilled nursing care and related services for residents who require medical or nursing care;
b. Rehabilitation services for the rehabilitation of injured, disabled or sick individuals; and
c. Health-related care and services to individuals who, because of their mental or physical condition, require care and services that are above the level of room and board and that can be made available to them only through an institution.
(g) "Primary caregiver'' means the family member or other person who normally provides the home care and supervision of the individual with ADRD.
(h) "Residential care facility" means a facility providing assistance with personal and social activities at one of the levels of care described in RSA 151:9, VII and VIII.
(i) "Respite care provider" means the entity chosen by the primary caregiver to provide ADRD respite care services, which is either:
(1) An agency providing health and/or social services; or
(2) An individual employed by such an agency.
(j) "Respite care service" means "respite care service" pursuant to RSA 161-F:66, IV namely "care provided on an intermittent basis to the eligible person to relieve the caregiver from the demands of the home care for a limited period of time."
(k) "Person-centered respite care plan" means a plan where the primary caregiver is the center of the system of care and his or her needs and preferences drive the development and implementation of the respite care services provided.
(l) "Wait list" means a list of individuals with ADRD maintained by the contract agency who have been determined eligible, and are ready to receive ADRD respite care services from a contract agency, but the agency does not have sufficient service units or resources to serve these individuals.
