Section He-A 406.04 - Request for an Administrative Hearing(a) A provider shall have 30 calendar days after receipt of the notice of a termination or suspension to request in writing a hearing to contest the action.(b) If a written request for a hearing is not received pursuant to (a) above, the provider waives his right to a hearing and the action of the department shall become final.(c) Hearings under this section shall be conducted in accordance with RSA 541-A and He-C 200.N.H. Admin. Code § He-A 406.04
