Section Gen 407.03 - Non-Academic Courses, Seminars, Conferences and Workshops, Including Grand Rounds Documentation of successful completion of non-academic courses, seminars, conferences and workshops including grand rounds shall be a photocopy of an official certificate showing:
(b) The name and location of the course, seminar, conference, workshop, or grand rounds;(c) The name of the provider or sponsoring entity;(d) The beginning and ending date(s) of the program, course, seminar, conference, workshop, or grand rounds;(e) The number of hours credited by the provider or sponsoring entity or the brochure showing the scheduled hours; and(f) The signature or stamp of the speaker, the instructor or a representative of the provider or sponsoring entity.N.H.
