Section Fis 1505.05 - Required Disclosure Statement(a) Pursuant to RSA 215-A: 26-a and RSA 215-C: 46, each licensed rental agent shall, prior to delivery of a rented snowmobile or OHRV, provide the renter with a department-supplied "OHRV/Snowmobile Rules Disclosure Statement" form, and: (1) The rental agent shall insert the name of the licensed rental business, its business address, and the name of the renter; and(2) The renter shall legibly sign and date the document.(b) One copy of the statement form shall be retained by the rental agent for a period of 3 years, one copy shall be provided to the renter, and one copy shall be remitted to department headquarters at 11 Hazen Drive, Concord, NH 03301 within 30 days of the transaction.N.H.
