Section Fis 1109.02 - Forms

Whenever a person is required to complete a written or electronic form, the following shall apply:

(a) Forms shall be filled out completely;

(b) Forms shall be completed in a legible manner; and

(c) All statements made on the form shall be true to the best information and belief of the person making the statement.
