Section Env-Wq 809.01 - Sludge Quality Certification ApplicationTo apply for a sludge quality certification, the generator shall provide 2 copies of the following to the department:
(a) The name and address of the facility, such as a wastewater treatment plant or other facility, that has generated or will generate the sludge;(b) The name, title, and telephone number of the operator of the facility;(c) If the generator is a POTW, a list and brief description of all industrial permit holders and each permit holder's local limits;(d) If the generator is an industrial wastewater treatment facility, the common chemical name of all chemicals being discharged to the treatment facility;(e) The quantity of sludge, in dry tons, generated annually by the generating facility for the previous 2 calendar years;(f) If the sludge is not generated in New Hampshire, certification that the sludge meets the chemical standards for land application in the state of origin as required by RSA 485-A:5-d;(g) If the sludge contains human waste, a description of the process to achieve class A or class B pathogen reduction requirements and vector attraction reduction requirements, including a citation to the applicable 40 CFR part 503 section; and(h) A sludge quality report that includes the following: (1) A description of the generating facility and sludge treatment process;(2) A chronological summary of analytical data from the previous 3 years, if available, for each detected chemical, presented in tabular form and by category; and(3) The results of testing required by Env-Wq 809.05, including laboratory reports, presented categorically as in (2), above, and summarized in tabular form.N.H.
