Section Env-Wq 304.12 - Signature Required(a) An applicant for certification, certification renewal, reciprocal certification, or retesting shall sign and date the application form.(b) The applicant's signature shall constitute certification that: (1) The information on the application form is true, complete, and not misleading to the best of the applicant's knowledge;(2) The applicant understands that: a. The submission of false, incomplete, or misleading information is grounds for denying the application or revoking any certification that is issued based on the information; andb. That he or she is subject to the penalties specified in RSA 641:3, as reprinted in Appendix C, for making unsworn false statements; and (3) If certification is granted, the applicant agrees to comply with Env-Wq 304.N.H. Code Admin. R. Env-Wq 304.12
