Section Env-Wq 1409.01 - Requests for Waivers of Minimum StandardsEach applicant for a waiver under RSA 483-B:9, V(i) shall provide the following information with the application required by Env-Wq 1406.06:
(a) A statement of the waiver requested, with specific reference to the paragraph or subparagraph of RSA 483-B:9, V for which the request is being made;(b) An explanation of how the applicable criteria of Env-Wq 1409.02 have been met;(c) If the request is for a reasonable accommodation of the needs of an individual with one or more disabilities, a statement signed by the physician who is attending the individual for the disability or disabilities certifying that the impacts or structures for which the waiver is being requested are necessary to accommodate the individual's disability or disabilities; and(d) Verification that all of the abutters have been notified of the proposed project as required by Env-Wq 1406.13.N.H. Code Admin. R. Env-Wq 1409.01
