N.H. Admin. Code § Env-Sw 1605.02

Section Env-Sw 1605.02 - Application Required

Any individual who is required to be certified pursuant to Env-Sw 1602.01 shall file an application that includes:

(a) The completed, signed application form as specified in Env-Sw 1604.01(b); and

(b) The fee described in Env-Sw 1605.04.
