Section Env-Dw 502.06 - Signatures and Certifications Required(a) The applicant shall sign and date the application.(b) If verification of employment is made on the application form, a responsible official shall sign and date the form where indicated and print his or her name and title and the name of the employer.(c) If verification of employment is made separately pursuant to Env-Dw 502.05(h)(2), a responsible official shall sign and date the statement and print or type his or her name and title and the name of the employer.(d) The applicant's signature shall constitute certification by the applicant that: (1) The information contained in or otherwise submitted with the document is true, complete, and not misleading to the best of the applicant's knowledge and belief; and(2) The applicant understands that: a.The submission of false, incomplete, or misleading information constitutes grounds for the department to: 2.Revoke any certification that is granted based on the information; and3.If the applicant is a professional engineer, refer the matter to the board of professional engineers established by RSA 310-A:3; andb.The applicant is subject to the penalties specified in New Hampshire law for falsification in official matters, currently RSA 641.(e) Each signature provided pursuant to (b) or (c), above, shall constitute certification by the signer that the information provided by or being certified by the signer is true, complete, and not misleading to the best of the signer's knowledge and belief.N.H.
