Section Env-Dw 305.27 - Final Report: Cover Page The cover page required by Env-Dw 305.26(c)(1) shall contain the following:
(a) Information to identify the persons involved, as follows: (1) The applicant's name, mailing address, daytime telephone number including area code, and website URL, if any;(2) The name, daytime telephone number including area code, and email address, if any, of an individual authorized by the applicant to represent the applicant for purposes of the department's review of the preliminary report;(3) The name, mailing address, email address, and daytime telephone number with area code of the water system owner and the owner of the property on which the well is located, if other than the applicant; and(4) The name, mailing address, daytime telephone number including area code, and email address of the person responsible for performing the pumping test and water quality sampling program; and(b) If any of the information provided pursuant to Env-Dw 305.06(b) or (c) has changed, updated information.N.H.
