Section Env-Dw 302.06 - Preliminary Report: Cover Page The cover page required by Env-Dw 302.05(a)(1) shall contain the following:
(a) Information to identify the persons involved, as follows:(1) The applicant's name, mailing address, daytime telephone number including area code, and website URL, if any;(2) The name, daytime telephone number including area code, and email address, if any, of an individual authorized by the applicant to represent the applicant for purposes of the department's review of the preliminary report;(3) The name, mailing address, email address, and daytime telephone number with area code of the water system owner and the owner of the property on which the well is or is proposed to be located, if other than the applicant; and(4) The name, mailing address, daytime telephone number including area code, and email address of the person engaged by the applicant to perform the pumping test and water quality sampling program;(b) A description of the purpose of each well for which approval is being sought and who is or will be served by the well; and(c) The proposed permitted production volume for all new sources of water for the CWS.N.H.
