Section Env-Dw 1408.04 - Signature Required(a) The authorized representative of the applicant or recipient that is requesting the waiver shall sign the request.(b) The authorized representative's signature shall constitute certification that: (1) The information provided is true, complete, and not misleading to the best of the signer's knowledge and belief;(2) The signer understands that any waiver granted based on false, incomplete, or misleading information is subject to revocation; and(3) The signer is subject to the penalties specified in New Hampshire law for falsification in official matters, currently RSA 641.(c) If the signer is or is acting on behalf of a listed engineer as defined in Env-C 502.10, the signature also shall constitute certification that the signer understands that the submittal of false, incomplete, or misleading information could constitute grounds for debarring the listed engineer from the roster.N.H.
