Section Env-A 607.03 - Applications for Temporary Permits(a) The owner or operator of any source or device required to obtain a temporary permit pursuant to Env-A 607.01 shall file the original and one copy of an application package as specified in (b) and (c), below, with the department.(b) The application package shall include the following:(1) The forms and information for the specific source or device as specified in Env-A 1703 through Env-A 1708, as applicable;(2) The fee(s) specified in Env-A 700, as applicable;(3) For a major source, all information specified in Env-A 1709;(4) For a major source of hazardous air pollutants subject to 112(g) of the Act, all information required pursuant to 40 CFR §63.43 (e);(5) If an air pollution dispersion modeling impact analysis is required pursuant to Env-A 606.02, documentation required pursuant to Env-A 606.04;(6) For a source subject to RSA 125-C:10-b, the information required pursuant to Env-A 624;(7) Certification of right, title, and interest and, if applicable, proof of good standing, as specified in Env-A 605.02;(8) Proof of legal authority as specified in Env-A 605.03, if the applicant is not the legal owner or operator of the source or device that is the subject of the application;(9) For a source with air pollution control equipment as defined in Env-A 101.17, an air pollution control equipment monitoring plan or catalyst management plan pursuant to Env-A 810.01; and(10) If a plan is not required pursuant to Env-A 810.01, a description of the monitoring the source intends to conduct to demonstrate compliance with all applicable state and federal statutes, rules, and permits, as specified in Env-A 810.02.(c) The information submitted pursuant to (a) and (b), above, shall contain enough detail for the department to reach its own conclusions regarding whether the application meets the requirements specified in Env-A 607.04.N.H.
