Section En 509.04 - F-22 Information Sheet(a) Each utility shall file department "Form F-22, Information Sheet," dated October 2023 with the department and the commission:(2) Whenever any changes occur to the information included in the Form F-22 filing.(b) Each utility shall include the following on Form F-22:(1) The name of the utility filing the report;(2) Person's name, title, and e-mail address to receive the annual report form;(3) Person's name, title, and e-mail address to receive the utility assessment tax;(4) The names and titles of the principal officers of the company; and(5) The signature, full name and title of the utility employee who supervised the preparation of the report, and date of signature.N.H.
