Section 695D.070 - Submission; format1. An organization that applies for a certificate of authority must submit to the Commissioner an original and two copies of its application.2. The original and each copy of the application must be submitted in a binder having three rings. The original application, the original of the completed forms supplied by the Division and the originals or certified copies of any supporting documents must all be contained in a single binder.3. Each binder must contain a table of contents and dividers that separate the various sections of the application and indicate the subject of each section.4. If an applicant submits any new pages to supplement or amend its application, the date of submission must be noted on the bottom of each page, and each page must be prepared so that it may be placed in the binder with the other materials.Nev.
