"Ultimate 1980 CSO Mortality Table" means the 1980 CSO Mortality Table, without the Ten-Year Select Mortality Factors, incorporated into the 1980 amendments to the Standard Valuation Law approved by the National Association of Insurance Commissioners in December 1983. A copy of the Ultimate 1980 CSO Mortality Table may be obtained from the Research Library of the National Association of Insurance Commissioners, 1100 Walnut Street, Suite 1500, Kansas City, Missouri 64106-2197 or by telephone at (816) 783-8250, for the price of $10, plus $1 per page.
