Section 687B.130 - Replacement of contract: Notice from insurer not using direct-response solicitation1. Upon determining that a sale will involve the replacement of a long-term care insurance contract, an insurer, other than an insurer using direct-response solicitation methods, or its agent, shall furnish the applicant, before the issuance or delivery of the individual long-term care insurance contract, a notice regarding the replacement of the policy for accident and sickness or long-term care coverage. One copy of the notice must be retained by the applicant, and an additional copy signed by the applicant must be retained by the insurer.2. The notice required by subsection 1 must be provided using form NDOI-954, which is available from the Division, or a similar form approved by the Commissioner.Nev. Admin. Code § 687B.130
