Section 679B.486 - Petition for declaratory order or advisory opinion: Filing and contents; limitation1. A petition which requests the Commissioner to issue a declaratory order or advisory opinion must be filed with the Commissioner.2. The petition must include:(a) The name and address of the petitioner;(b) The reason for requesting the order or opinion; and(c) A clear and concise statement of the question to be decided by the Commissioner and the relief sought by the petitioner.3. The Commissioner will not review a petition filed by an interested person which requests him or her to issue a declaratory order or an advisory opinion concerning a question or matter that is an issue in an administrative, civil or criminal proceeding in which the interested person is a party.Nev.
