Section 645C.050 - Application for license or certificate: Required information; period of validityThe application for a license or certificate as an appraiser provided by the Division requires the following information:
1. The name of the applicant;2. The social security number of the applicant;3. One recent photograph of the applicant;4. The information required of the applicant pursuant to NRS 645C.290; and5. Such other pertinent information as the Division may require. w The application must be accompanied by a completed fingerprint card as required by NRS 645C.300.Nev. Admin. Code § 645C.050
