Section 640E.110 - Temporary license: Limitations; application1. Any temporary license issued pursuant to NRS 640E.190 must be limited to the providing of medical nutrition therapy in this State.2. An application for a temporary license must include the following information:(a) The applicant's name, the jurisdiction in which the applicant holds a corresponding license, the number of that license and the applicant's registration number from the Commission on Dietetic Registration or its successor organization;(b) A declaration that the license is in good standing in the jurisdiction in which it was issued;(c) The name of the entity for which the licensee will be providing services; and(d) The address at which the licensee will be providing services.Nev.
