Section 640A.260 - Supervision and employment of occupational therapy assistant: Verification; notice of termination1. An occupational therapy assistant or provisional licensee shall submit verification of his or her employment and supervision by a primary supervisor to the Board within 30 days after a change in the employment or primary supervisor. The verification must be submitted in a format approved by the Board.2. An occupational therapist who is a primary supervisor shall notify the Board within 30 days after the termination of his or her supervision of an occupational therapy assistant or provisional licensee.3. An occupational therapy assistant or provisional licensee must have at least one primary supervisor and may have one alternate primary supervisor for each employer of record.Nev. Admin.
