"Primary professional address" means:
1. The physical address where a licensee practices physical therapy or carries out any other activities relating to physical therapy for the majority of the working hours of the licensee within a consecutive 30-day period; or2. If a licensee does not actively practice physical therapy, the mailing address designated as the primary professional address by the licensee.Nev. Admin. Code § 640.Sec. 7
