Section 640.020 - Application for license1. Each completed application for a license as a physical therapist or a physical therapist assistant must:(a) Be accompanied by the appropriate fee; and(b) Be verified by oath of the applicant.2. Each applicant must: (a) Attach to the application, in a sealed envelope, the official transcript of the applicant from a school of physical therapy or request that the school provide the transcript directly to the Board.(b) Request the Federation of State Boards of Physical Therapy to provide directly to the Board the scores which the applicant received on the examination required by NRS 640.080.(c) Successfully complete the jurisprudence examination administered pursuant to subsection 2 of NAC 640.040.4. Each applicant who currently holds or previously held a license in another state or territory and who is applying for a license in this State must provide to the Board proof of the licensure of the applicant in each state or territory in which the applicant currently holds or previously held a license. The applicant must request that the licensing board of each state or territory where the applicant currently holds or previously held a license provide directly to the Board proof of the licensure of the applicant in that state or territory.Nev.
