Section 638.535 - Records of receipt and use of sodium pentobarbital
1. An agency or a euthanasia technician shall keep a record of all euthanasic agents received and used by the agency.
2. The record must contain:
(a) A weekly verification of the stock of euthanasic agents, signed by the euthanasia technician.
(b) A notation of the date and amount of any euthanasic agent in a container each time that the euthanasic agent is removed from secure storage.
(c) The species and weight of each animal to which a euthanasic agent is administered.
(d) The amount of euthanasic agent used for each administration.
(e) The name or initials of the euthanasia technician who administered the euthanasic agent.
(f) A notation of any waste of a euthanasic agent, signed by the euthanasia technician who administered the euthanasic agent.
(g) A notation of the disposal of any expired or unwanted euthanasic agent.
3. A euthanasia technician shall file all records in chronological order in a binder that is labeled with the name of the agency. The binder must remain at the agency and be available to the Board or its representative for inspection.
4. An agency shall keep all records for not less than 4 years.
