Section 634.370 - Replacement of lost, stolen or mangled license or certificate

A licensee or holder of a certificate is entitled to a duplicate license or certificate if the original license or certificate is lost, stolen or mangled. Each request for a duplicate license or certificate must be accompanied by the required fee and a signed written statement which sets forth the circumstances concerning the need for the duplicate license or certificate. If the duplicate license or certificate is requested to replace a mangled license or certificate, the licensee or holder of the certificate shall submit the mangled license or certificate with the request.
