The following fees are established:

1. For a registered nurse:

NCLEX Verification or the National Practitioner Data Bank Verification......................	5.00
Application for a license to practice as an advanced practice registered nurse .........................................................................................................................	200.00
Application for certification as a certified registered nurse anesthetist........................	200.00
Biennial fee for renewal of a license to practice as an advanced practice registered nurse or certification as a registered nurse anesthetist.............................	200.00
2. For a practical nurse:

3. For a registered nurse or practical nurse:
