Section 623A.145 - Lost, stolen, mutilated or destroyed certificate: Notification of Board; issuance of duplicate1. A certificate holder shall notify the Board in writing if his or her certificate of registration or certificate to practice as a landscape architect intern is lost, stolen, mutilated or destroyed.2. The Board will issue a duplicate certificate of registration or certificate to practice as a landscape architect intern upon request of a certificate holder which is accompanied by a signed affidavit detailing the loss, theft, mutilation or destruction of the original certificate, and upon payment of the fee for the issuance of a certificate of registration as provided in NAC 623A.170.Nev.
