Section 618.6343 - Request for hearing1. Any affected employer or employee may file with the Administrator a request for a hearing on an application for a variance.2. A request for such a hearing must include:(a) The name, address and telephone number of the person requesting the hearing;(b) A concise statement of facts showing how the employer and employee would be affected by the variance applied for;(c) An identification of any statement or representation in the application which the person requesting the hearing denies, and a concise summary of the evidence that the person would adduce in support of each denial; and(d) Any statements concerning any issue of fact or law raised in the application.Nev.
